Attention,

Si votre enfant est inscrit à l’UAPE ou à l’ECOLINE :  

Vous devez leur envoyer un e-mail autorisant un moniteur du taekwondo à récupérer votre enfant pour le cours de Taekwondo en précisant les mêmes heures et jours indiqués dans ce formulaire.

Nous ne pourrons pas récupérer les enfants qui n’ont pas envoyé d'autorisation à l’UAPE ou l’ECOLINE

A la sortie de l’école à 15h15 :

Nos professeurs et moniteurs attendront vos enfants à la sortie principale de l’école entre 15h15 et 15h30 donc vous devez bien expliquer à vos enfants :

1. Qu’en « aucun cas » ils quitteront l’école tous seuls.
2. Que s’ils sortent de l’école en retard après 15h30, ils doivent attendre à la sortie jusqu’à ce qu'on vienne les chercher.

Si un enfant doit être récupéré à la sortie de l’école et qu'il n'est pas là, on appelle par téléphone le contact inscrit dans ce formulaire, s’il n’y a pas de réponse après 3 essais, on appelle la police.  

Merci de nous informer minimum un jour en avance si un enfant ne vient pas au cours de Taekwondo.

If your child is enrolled at the UAPE or ECOLINE :  

You must send them an e-mail authorizing a Taekwondo instructor to pick up your child for the Taekwondo class, specifying the same times and days indicated on this form.

We will not be able to collect children who have not sent an authorization to the UAPE or ECOLINE.

On leaving school at 3.15pm:

Our teachers and monitors will be waiting for your children at the main school exit between 3.15 and 3.30 p.m., so please explain to your children:

Under no circumstances will they leave the school on their own.
That if they leave school late after 3:30 p.m., they must wait at the exit until they are picked up.
If a child needs to be collected from school and is not there, we will call the contact person listed on this form. If there is no answer after 3 attempts, we will call the police.  

Please inform us at least one day in advance if a child does not come to Taekwondo class.
